SSA Action Log

Small actions can make a big difference. Record a time you used your SSA skills to include, support, or safely help someone.

Keep it brief. Keep it respectful. Focus on what you noticed and how you helped. Leave out other students’ names, initials, and identifying details. Entries are saved for staff review and are not displayed on this page.

Need help now? This log is not monitored continuously. If someone is unsafe, a situation is escalating, or you need an adult’s help, speak directly to a trusted adult right away. Stay safe and use the skills you practiced in SSA.

Record your action

Complete one entry for each situation. Fields marked with an asterisk (*) are required.

Think ahead: What is one small action you can take tomorrow to help someone feel included?

How to use this SSA action log

SSA action log: directions & next steps

Use the SSA action log to reflect on a safe action you took as a Safe School Ambassador. Read the current directions on this page before you begin, because the activity or available choices may change.

Before you begin

Think about one situation at a time. Identify what you noticed, where it happened, and which safe action you chose.

Work through the task

Describe your own observations and actions briefly. Follow the privacy reminder above and leave out other students’ names and unnecessary personal details.

Check your work

Distinguish what you saw from what you assumed. Explain the outcome you actually noticed, and use the adult-help and follow-up fields honestly.

What happens next?

This form is a reflection record. For an urgent safety concern, speak directly with a trusted adult instead of waiting for a form response.
